Investigators who have already worked with the Biorepository, and submitted the necessary forms for new investigators, may use the forms below.

BIOR Request of Storage Client Specimens

Request specimens stored under your IRB-approved protocol with this form.

Specimen Destruction Request Form

You’ll need to click “Add document” on the next screen to access this Sharepoint form. You may attach Excel sheets here if needed.

Specimen Relabeling Request Form
